If so a vacancy has arisen for a part-time (30 hours per week) Assessment & Support Co-ordinator required to work in our busy Christchurch Locality Team.

Previous experience of working in an Adult Social Care setting is desirable, but more importantly you will be a good team player, IT literate and demonstrating enthusiasm for working with people.

Under the direction of the Area Practice Manager you will work with older people and people with physical disabilities undertaking assessments, support planning and reviews of Care Packages.

There is a comprehensive induction programme as well as ongoing support, training and development opportunities within this role.

This post has a significant travel requirement and you are contractually required to have a vehicle (or transport deemed to be suitable by the County Council) available for use as required.
