We are looking for an experienced and motivated part-time (24 hours) Area Practice Manager to join our busy dynamic hospital discharge team within the Adult Social Care Team. 

This is an exciting opportunity as we work towards continuous development within integrated working in health and social care services enhancing timely discharge and admission avoidance planning for our customers. 

We are looking for a strong people leader with experience of working with adults (older people and adults over the age of 18) Ideally you will need experience of working in an acute hospital setting ,however, applications from candidates with transferable skills who are motivated to undertake a new challenge may be considered but you will need an understanding of hospital discharge and working within a busy acute hospital setting within an multi- disciplinary team to assist in facilitating safe, effective and appropriate discharge from hospital.

You will be based at the Royal Bournemouth Hospital but also required to travel and support other hospitals within Dorset or on Dorset's Borders who provide a service to Dorset customers.

Your role will be to support the Area Manager, working with the current Area Practice Manager in post you will work together to lead and manage a team of Social Workers, Assessment Support Co-ordinators and new staff members. Expectation will be to oversee casework, coach individuals to support their professional development, develop sound decision-making and confident actions. You will also need a good understanding and knowledge of budget management, quality and performance standards, and development of services, integrated working and alternative resources in discharge planning.

You must have a degree in Social Work or equivalent, be registered with the HCPC and be able to fulfil the travel requirements of the post.
