Dorset County Council have a 22.5 - 25 hour vacancy for an experienced Social Worker to manage Dorset residents discharges from Salisbury District Hospital.

In this new and exciting position we are seeking an experienced Social Worker to support discharges from Salisbury District Hospital completing Care Act assessments when required. The role will involve working closely with GP practices, the existing full time Salisbury Social Worker, preventing admission and working closely with communities and all partners.

Experience of hospital discharge and working in an integrated health setting is an advantage but not essential. This post is open to applicants who have transferable skills and would like to undertake a new challenge.

You will be well supported by the existing teams and your line manager in developing both your own skills and the role. You will be linked into the Dorset Social Care Team based in The Royal Bournemouth Hospital Team for additional support but will be mostly working flexibly in Dorset. There will be an emphasis on working closely with the Emergency Department on admission avoidance.

You must have a degree in Social Work or equivalent, be registered with the HCPC and be able to fulfil the travel requirements of the post.
